How to Report a Claim
Old Glory provides its policyholders with several options for filing
workers’ compensation claims.
- Call Old Glory Insurance Company and speak to a claims representative at
800.765.2412.
- Download the appropriate First Report of Injury (FROI) below
and fax the completed form to 903.509.1888.
- Report your claim online. Login at left, go to Administration > File Claims
and select the applicable form for your claim. Contact Old Glory Insurance Company at 903.509.4929 or
888.838.6404 to setup your username and password.
